Introduction

Passport is an important document that has to be kept ready, especially while travelling outside India. This official document is issued by the government and it allows the holder to travel outside the country without any trouble. It helps in identifying an individual and his/her nationality during international travel. It can also be used as an identity and address proof.

If you are looking to get a passport for yourself, the first step is to make an application and arrange all the required paperwork. Although passport application has now become seamless, a few passport applications could be rejected for different reasons. This makes it even more essential to be aware of the passport application process and what consists of the application form. Read on to find out all the necessary details of the passport application form.

What is the process to fill the passport application form?

There are certain rules to be followed while filling a passport application form. These apply to the main and also the supplementary form. Since passport application forms are machine-readable (through ICR scanners), if these are not filled appropriately, these will not be read correctly and could be rejected. Information that has to be included in your passport will be as per what is filled in the form. Therefore, make sure to provide clear and accurate information on the same. It is also considered as a criminal offence if you deliberately provide incorrect or incomplete information or have withheld essential information.

Online Form Submission

  • The first step is to register yourself on the Passport Seva Online Portal. (Use the"Register Now" link available on the Home Page).
  • Login to the Passport Seva website by using the registered Login Id.
  • Click on "Apply for Fresh Passport/Re-issue of Passport" option.
  • Enter the required details in the application form and click on submit.
  • Use the "Pay and Schedule Appointment" link available on the "View Saved/Submitted Applications" page for scheduling an appointment.
  • Online Payment is mandatory to proceed with booking appointments with all PSK/POPSK/PO. Online Payment can be done through either of the following modes:
    • Credit/Debit Card (MasterCard and Visa)
    • Internet Banking (State Bank of India (SBI) Associate Banks and Other Banks)
    • SBI Bank Challan
  • Click the "Print Application Receipt" link for printing the application receipt which contains Application Reference Number (ARN)/Appointment Number. Printout of Application Receipt is not required any longer. Showing SMS that contains your appointment details is now accepted as proof of your appointment while visiting the Passport Office.
  • Visit the Passport Seva Kendra (PSK)/Regional Passport Office (RPO) as per your appointment booking. Do carry your original documents.

Offline Form Submission

Guideline for offline form filling and submission is as follows:

  • All the details in the form must be filled in Capital letters and each box should contain only one letter
  • Ensure to use a ball-point pen in blue or black colour. Avoid using an ink pen
  • Fill in only required details and leave the non-applicable sections blank. Do not strike out or mark as NA in case a field doesn't apply to you.
  • Avoid folding or dirtying the form
  • Passport Photograph - Do not stick a photograph if the form has to be submitted at a PSK. If you are submitting the form at other centres such as DPCs, SPCs or CSCs, you will need to paste a photograph as per the following dimensions and specifications:
    • Unsigned photograph
    • Only one photograph has to be pasted which fits in the space given, neither big nor small
    • Latest colour photograph
    • Frontal view of the applicant, head centred with both ears and facial edges should be visible, eyes must be open. The photograph should have a neutral facial expression
    • White background with a dark outfit (no uniforms are permitted)
    • Dimensions - 4.5 cms X 3.5 cms
    • Use good quality paper for the photo. Computer prints are not allowed, damaged/torn photos will be rejected.
    • The applicant must sign under the passport within the box given. In case of minor applicants, the minor’s signature/thumb impression is required. Parents/guardians cannot sign on behalf of the minor. Signature/thumb impression has to be within the box and must be accurately scanned.

Important Things to Remember while Filling the Passport Application

Here are a few crucial points to be kept in mind while filling an application form:

  • All the information filled should be in capital letters and maintain 1 letter per box rule. The form should be filled and there should be space between two words
  • Your name must be mentioned very clearly with one letter per box. Ensure to fill in the given name or surname. Every word has to be separated by leaving a blank box between two words.
  • An ink pen should not be used. Use a ballpoint pen in blue or black colour.
  • The application form should not be folded, dirtied or smudged.

1. What are the things I need to take care of while applying for a passport online?

For an online passport application, you must first get yourself registered. You can also download an e-Form which can be filled offline and uploaded later. 4. What documents do I need to submit while applying for a passport in tatkal mode?

Here are the documents that you need to submit while applying for a passport in the tatkal mode:

  • Proof of present address.
  • Proof of date of birth.
  • Prior Intimation letter as per Annexure H.

5. Is it necessary to carry a printout of the passport application form for re-issuance?

No, you don’t have to carry the printout of the passport application form while visiting the passport office as per scheduled appointment. An SMS will be sent on your registered mobile number and this is accepted as proof of appointment.
